More Than Choosing a Treatment

Cancer treatment is becoming increasingly personalized.

Cancer cells can continue to evolve during disease progression and treatment. Different cells within the same tumor may also have different molecular and immunological characteristics.

For this reason, treatment decisions should not be based solely on the name or type of cancer.

The aim is not simply to use as many different immune cells as possible.

Why Is an Individual Assessment Important?

One of the major challenges of cancer is its ability to change over time.

Tumors can contain considerable genetic and epigenetic heterogeneity. During natural disease progression and under the selective pressure of treatment, some cancer-cell populations may acquire characteristics that allow them to survive or become resistant.

A treatment that was initially effective may therefore not necessarily control the disease indefinitely.

Similarly, even when imaging shows significant tumor regression or no detectable disease, medical imaging has limits of detection and cannot by itself exclude the presence of all microscopic or residual cancer cells.

This is one reason why ongoing surveillance and reassessment can remain important even after a good treatment response.

Cancer treatment is not always a single decision—it is an evolving strategy that may need to change as the disease changes.

Reducing the Risk of Cancer Recurrence

For patients who have completed their primary cancer treatment and currently have no detectable disease, the focus of consultation can be different.

Rather than treating measurable tumors, we may discuss longer-term strategies involving:

Management of modifiable health and recurrence risk factors
Immune health and immune surveillance

T cells and NK cells play important roles in normal immune surveillance of abnormal cells. For selected individuals, we can discuss the rationale, limitations and available evidence for preventive immune-cell approaches as part of a broader long-term management strategy.

Preventive immune-cell therapy cannot guarantee that cancer will not recur and should not replace evidence-based cancer surveillance, screening or standard follow-up care.

Surgery, chemotherapy, radiotherapy, targeted therapies, immune checkpoint inhibitors and cellular immunotherapies each work through different mechanisms and have different indications.

For some patients, standard treatment remains the most appropriate option. For others, it may be reasonable to explore whether personalized immune-cell therapy could complement their existing treatment strategy.

We therefore focus on understanding the individual patient first, rather than recommending the same treatment to everyone.
